New Computer Systems Registration

This form is for the registration of new computer systems users want configured for the Martinos NMR Center operating environment

CRITICALLY IMPORTANT! YOU MUST CONSIDER BACKUP!

The following four questions need to be answered to your satisfaction.

  • Where is backup taking place? The IT group is only responsible for backing up central systems.
  • If you do your own backup, what media will be used? There are options so please ask.
  • How often will you do backup? We have examples so please ask.
  • If you are not doing backup, are you comfortable with losing everything?

[1] Include PO# if bought by PO, fund used if cash reimbursal, institute bought by or transfered from if not Partners, or just put PERSONAL if you own it personally. WE NEED SOME KIND OF TAG LIKE A PO# TO IDENTIFY MACHINES SHIPPED TO US. If we cannot ID a shipment, setup of your system will be delayed.
